Op amp is packaged in 14-SOIC (0.154, 3.90mm Width) case. A General Purpose-type buffer amplifier is shown here. There is a Cut Tape (CT) case that is delivered with the instrumentation amplifiers. In order to be more specific, this instrumentation amplifier is one of the types of Operational Amplifier devices. There are 14 pins on this chip.
